New Patients

Our practice currently has an open list and we are able to register new patients who are living within our designated practice area.


You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

How to register

To complete your registration we require the following:

  • NHS Card
  • New Patient Registration Pack
  • Proof of Address (dated within the last 3 months)
  • Proof of Identity

NHS Card

Your NHS card contains your NHS number and details of your previous GP. If you cannot find your NHS card you can find out your NHS number by contacting your previous GP surgery.

New Patient Registration Pack

The New Patient Registration pack can be obtained from our Reception or you can download a PDF version from this web-site. The pack includes:

  • GMS1 Registration Form
  • Health Questionnaire
  • Alcohol Questionnaire
  • Ethnicity Form
  • Contact Authorisation Form
  • Patient Access Application Form

New Patient Registration Pack – CHILD under 16

Proof of Address

Proof of address documentation examples (must be dated within the last 3 months):

  • Utility bill (gas, electricity, community charge etc)
  • Phone bill stating address
  • Credit card/bank statement
  • Rent book or tenancy agreement
  • Pension book
  • Benefit/family credit book
  • Home office permit to stay
  • Document showing University/college hall of residence
  • TV license
  • Driving license (with address)
  • HM Revenue and Customs Statements
  • Council Tax Bill/Council Rent book
  • Home Insurance Policy
  • Documentation from a reputable source, for example a letter from a voluntary organisation or a refuge.

Your address must be within our designated Practice Area.

Proof of Identity

This can be one of the following:

  • Passport
  • ID Card
  • Photo Driver’s License (UK only)

For all children please also provide the Red Book. This contains important information about your child’s immunisation history and developmental records.

PLEASE NOTE: Children (under 18 years) can only register if a parent or legal Guardian is also registered.
