How do I get a Sick Note?
A GP certificate is not required for illness up to seven days including weekends. Please instead use an employer's form or an SC2 self-certification form which you can get from a Post Office or from www.hmrc.gov.uk . Further information is available on our page on Medical Certificates.

How do I get a Home Visit?
Home visits are available for patients who are very ill and unable to attend the surgery. To request a home visit phone Reception on 020 8441 4425 before 10am. You may be asked to discuss your problem with a doctor before a visit is arranged.

How do I make a comment or complaint?
At the Addington Medical Centre we aim to give patients the best possible care and attention and we always welcome comments or suggestions. If you have a complaint or concern about the service you have received from the doctors or any of the staff working in this practice, please let the practice manager know. We operate a practice complaints procedure as part of the NHS system for dealing with complaints. Our complaints system meets national criteria.

How do I check whether I am fit to drive?
If you have had, or currently suffer from a medical condition or disability that may affect your driving you must tell the Driver and Vehicle Licensing Agency (DVLA). You'll also need to provide details if you develop a new condition or disability or one that has become worse since your licence was issued.
